If you are currently being cared for and want to receive a copy of your records please mail a request to the above address. If you are not currently in care and want a copy of your records, please mail a check for $15 to the above address.
When requesting records, please include the following sentence: "I release Dr. Rabin and his staff from any damages or HIPAA violations should my records not arrive where I have asked him to send them." just in case the post office fails us.
